Yes, we are accepting new patients. There may be times when a specific provider must limit new patients in order to provide care to current patients.
You may schedule your appointment online in our patient portal. You will need a valid email to schedule and register.
Please be advised there is a strict no-show and 24-hour cancellation policy provided in the new patient paperwork. The terms of the no-show and 24-hour cancellation policy are briefly explained below.
For your convenience, we offer Telehealth services if you are a resident in the state of Kentucky and if your insurance company covers the service. Please contact your insurance company to see if telehealth services are covered. If telehealth services are not covered by insurance, out-of-pocket fees would apply and are accepted. Please note that you must have a way to obtain vital signs for telehealth appointments. Vital signs are Blood Pressure, Heart Rate, Weight and Height.
If you are taking or prescribed controlled substance(s) you will need to be seen in person first before telehealth can be started for regular follow up appointments (see controlled substance FAQ below). The United States and the DEA require that patients on controlled substances must be seen in person by the provider at least one time a year after the initial in office visit. Regular drug screening every 3 months, and randomly, will be required and ordered. You will need to take the emailed drug screening lab order to your local lab and have the lab drawn when needed in order to continue controlled medications via telehealth. We offer in-office drug screening as well.
The telehealth regulations are a Federal Law. Further, if your provider validates that you need to be seen in the office vs. a telehealth appointment you must comply with your provider to continue care.
We take all Kentucky Medicaid, Cigna/Evernorth, Humana and Humana Medicaid, United Health Care, United Healthcare Community Plan, Wellcare.
*We are currently not credentialed with Medicare. Our goal is to start the credentialing process Fall of 2024.
In addition, we see out-of-pocket payers on an up-front payment option. Please request our list of “cash-only” fees by contacting our office.
All patients will be required to have an up-to-date and valid credit/debit card on file regardless of the use of insurance or not. The credit card will not be charged until the time of your appointment and/or for appointments cancelled less then 24 hours prior and for no-show appointments. Information explaining this request will be provided in the paperwork sent to you online prior to your first appointment.
Our providers are psychiatric nurse practitioners (PMHNP) who provide psychiatric evaluations, diagnosis and medication management. Our office does not have therapists, but we have working relationships with many therapy practices throughout the state and collaborate with those therapists to meet the needs of our patients. At times, it may be best for a patient to receive all care in one setting (such as case management along with therapy and medication management). In cases like this, a patient may require a referral outside of MindWell Mental Healthcare of Kentucky LLC for a higher level of care.
Our providers treat a variety of mental health disorders. Please refer to our services page on this website for a limited comprehensive list of mental health diagnosis treatment.
We do not do long-term or Government disability evaluations or handle worker’s compensation cases. We do not do Social Security disability evaluations. The Social Security department will have a provider hired by them to do these evaluations.
To schedule a new patient appointment please refer to our new patient screening form that you will receive upon scheduling your appointment in our patient portal. The screening form is required and necessary to insure that our providers are able to meet your potential mental health needs. It is our top priority that your requests are met, however this may not always be the case and you deserve to know as quickly as possible so that you may be able to find a care provider elsewhere to serve you.
When the screening form is filled out it will be reviewed for patient-provider compatibility. If we are able to meet your mental health needs, you will receive an email with the link to schedule your appointment and will then need to fill out all new patient paperwork and forms provided prior to your first appointment. The new patient paperwork will be sent to you via email and can easily be filled out online and returned. You will receive a phone call, text or email from our office informing you of your acceptance. The email will include the required new patient paperwork.
For pediatric patients, we require consent from all custodial parents or legal guardians. We require legal proof of guardianship and/or custodianship for the safety of the pediatric patient and HIPAA compliance. Our office does not have the capacity to do evaluations for custody, guardianship, or other forensic evaluations.

We do not prescribe long-term benzodiazepines and an initial visit with a provider does not mean current medications will be continued. Our office does not prescribe suboxone. Additionally, certain medications will not be prescribed if someone is drinking excessive alcohol or utilizing other substances such as marijuana or illegal substances. A medical marijuana waiver does not mean a provider will prescribe a controlled substance.
In Kentucky, a nurse practitioner is not able to provide a medical marijuana waiver. This must be done by an MD or DO in the state of Kentucky per executive order 2022-798.
Controlled substances require a controlled substance agreement with baseline, scheduled and random drug screens provided in our office. In many cases there may also need to be out of the office baseline diagnostics performed such as lab work or EKG - these are determined at the time of your appointment and based on individual medications and medical history. Additionally, if a patient is prescribed controlled substances, appointments will occur more frequently to be compliant with Kentucky laws related to the prescribing of controlled substances.
We have provided the Americans with Disabilities Act web page and PDF booklet defining the difference and qualifications for Service Animals (SA) and Emotional Support Animals (ESA). There are required qualifications within the ADA guidelines for specified handlers (owners) of ESA. There is not a feasible or safe option for a psychiatric provider to assess the listed qualifications of the animal or handler outlined by the ADA. If you are unable to keep an appointment, please call the office as soon as possible and/or reschedule on the scheduling portal located on our website. If it is less then 24 hours, our broken appointment/late cancellation policy will apply and you will be responsible for any fees incurred. The broken appointment/late cancellation fees are: $125.00 for new patients and $50.00 for existing patients. After three "no-show" appointments, our office will terminate the client/patient relationship and you will be notified online and/or by certified mail.
We understand it is a lot of paperwork to begin care. Please be aware that most forms will only need to be filled out once and/or updated yearly. All forms sent to you are required and necessary for us to provide you with the best treatment possible. The forms cover everything from office requirements and policy to your individual mental health history and your present mental health concerns. The forms allow us to review your concerns PRIOR to your appointment thus increasing the understanding of your needs and allowing our office and providers to serve you efficiently.
New patient packets MUST be completed entirely prior to your appointment. If the forms are not filled out and turned in prior to your appointment you will be asked to reschedule your appointment as the time taken to fill out the forms subtracts the necessary time from your appointment with the provider and other patients waiting for their appointments. In addition, our broken appointment/late cancellation policy will apply and you will be responsible for any fees incurred.
Please be patient with all of the necessary forms-once you are finished with them you are “good to go”!
