Yes! MindWell is currently accepting new patients with our provider, Michael Sarten, APRN, PMHNP. Please note that Michael is currently completing the credentialing process for our office with multiple insurance companies. If he is not yet in-network with your insurance plan, our office will be happy to discuss your options and provide updates on his credentialing status and make sure you are scheduled and on your way to better mental health.

Yes, we have both telehealth and in-office appointments at MindWell open for all Kentucky residents.
Important information regarding Telehealth services:
If you are a perminate resident in the state of Kentucky and if your insurance company covers the telehealth appointment. Please contact your insurance company to see if telehealth services are covered by your individual insurance plan. If telehealth services are not covered by your insurance, out-of-pocket fees would apply and are accepted.
Please note that you must have a way to obtain vital signs for telehealth appointments. Vital signs are Blood Pressure, Heart Rate, Weight and Height.

If one of our providers is considered out-of-network with your insurance plan, we are happy to provide a superbill that you may submit to your insurance company for possible reimbursement. In these situations, payment is due at the time of your appointment.
We also welcome self-pay patients. Please contact our office for our current self-pay fee schedule.
To simplify billing, all patients are required to keep a valid credit or debit card securely on file, regardless of insurance coverage or payment method. Your card will only be charged for your patient responsibility (such as copays or balances), self-pay appointments, late cancellations (less than 24 hours' notice), or missed appointments, in accordance with our office policies. Additional information will be provided in your new patient paperwork prior to your first appointment.
Please note: MindWell Mental Healthcare of Kentucky is not currently enrolled with Medicare.
Our providers are licensed and board-certified Psychiatric Mental Health Nurse Practitioners (PMHNPs) who provide comprehensive psychiatric evaluations, diagnosis, medication management, and ongoing treatment for children starting at 5 years old, adolescents, and adults.
While MindWell does not have therapists on staff, we have established relationships with many outstanding therapy practices throughout Kentucky. We believe therapy and medication management often work best together, and we regularly collaborate with therapists to provide coordinated, comprehensive care for our clients.
In some situations, a higher level of care or additional services—such as therapy, case management, intensive outpatient treatment, or community mental health services—may be the most appropriate option. If we believe another provider or service would better meet your needs, we will discuss our recommendations with you and help guide you toward the most appropriate level of care to safely meet your individual needs.

Please note: MindWell does not provide Social Security Disability, long-term disability, government disability, or workers' compensation evaluations.
Scheduling an appointment with MindWell is simple! New and established patients can schedule appointments online by visiting our Appointments page. If you prefer, you may also contact our office by phone to speak with our Office Administrator or leave a message through our secure voicemail system.
For New Patients:
After requesting an appointment, you will receive a brief screening questionnaire. This allows us to determine whether our providers and services are the best fit for your individual needs.
If it is determined that our providers or services are not the best fit for your needs, we will notify you using the contact information you provided. Due to the volume of requests we receive, we are unable to provide individualized referral recommendations for patients we are unable to accept.
If we are able to meet your mental health needs, you will receive an email with a link to schedule your appointment, along with your new patient paperwork. All required forms can be completed securely online and must be submitted before your first appointment.
If we believe another provider or level of care would better meet your needs, we will let you know and, whenever possible, provide recommendations for appropriate resources.
For Pediatric Patients:
For patients under the age of 18, consent is required from all custodial parents or legal guardians. When applicable, we require legal documentation of guardianship or custody, to ensure patient safety and compliance with HIPAA and Kentucky law.
Note: MindWell does not perform custody evaluations, guardianship evaluations, or other forensic mental health evaluations.

Controlled substances are prescribed only when deemed appropriate by your provider following a comprehensive psychiatric evaluation. Decisions regarding controlled medications are based on your safety, your diagnosis, medical history, current medications, applicable laws and regulations, and the provider's independent clinical judgment. Because patient safety is our highest priority, all controlled substances are prescribed in accordance with federal and Kentucky laws, current clinical guidelines, and the provider's professional judgment.
While we understand that many new patients come to us already taking controlled substances, continuation of these medications is not automatic and should not be assumed. All medication recommendations are made following a comprehensive psychiatric evaluation and are based on your individual clinical needs, current standards of care, and your provider's independent clinical judgment.
Patients who are prescribed controlled substances will be required to sign a Controlled Substance Agreement (CSA) and comply with all applicable office policies, federal and Kentucky laws, and provider recommendations.
Depending on the medication prescribed, requirements may include baseline and random urine drug screens (for patients ages 12 and older), random pill count requests, prescription monitoring through the Kentucky All Schedule Prescription Electronic Reporting (KASPER) system with each controlled substance prescription, and regular follow-up appointments every 1–3 months based on your individual treatment plan and prescription requirements.
Note: The monitoring requirements are intended to promote safe prescribing practices, are applied consistently to all patients receiving controlled substances, and to remain compliant with federal and Kentucky prescribing requirements and to ensure medications continue to be safe and effective.
Please be aware of the following office policies:
Some medications also require additional monitoring, such as laboratory testing, EKGs, or other diagnostic studies. These requirements are determined by your provider based on your medical history, current medications, and individual treatment plan. If testing is required, you will receive instructions during your appointment.
To help our patients better understand the differences between Service Animals and Emotional Support Animals (ESAs), we have provided links to educational resources below.
While we recognize that Emotional Support Animals can provide comfort and companionship for many individuals, MindWell Mental Healthcare of Kentucky does not provide Emotional Support Animal (ESA) letters.
Our decision is based on several factors. Determining whether an Emotional Support Animal is appropriate requires a comprehensive evaluation that extends beyond the scope of a routine psychiatric appointment and includes considerations outside the provider-patient relationship. In addition, our providers believe these determinations are best made within the context of an established, ongoing therapeutic relationship rather than a one-time evaluation performed solely for documentation purposes.

If you are unable to keep your appointment, please notify our office as soon as possible. You may also cancel or reschedule your appointment through the online scheduling portal on our website.
Appointments cancelled with less than 24 hours' notice, as well as missed ("no-show") appointments, are subject to our late cancellation policy. The current fees are:
These fees help offset the time that was reserved specifically for you and the opportunity to offer that appointment to another patient in need of care and waiting to be seen.
After three no-show appointments, MindWell Mental Healthcare of Kentucky reserves the right to terminate the provider-patient relationship. If this occurs, you will be notified through the patient portal and/or by certified mail, and provided with information for alternate providers outside of MindWell.
Payment is due at the time services are provided.

For patients using insurance, any copay, deductible, coinsurance, or other patient responsibility determined by your insurance company is due at the time of your appointment. Commercial insurance plans require healthcare providers to collect these amounts at the time services are rendered.
If our providers are considered out-of-network with your insurance plan, payment in full is due at the time of your appointment. Upon request, we will provide a superbill that you may submit to your insurance company for possible out-of-network reimbursement. Any reimbursement is determined by your individual insurance plan and is not guaranteed.
For patients choosing our self-pay option, payment is due at the time of your appointment.

We understand that completing your new patient paperwork can feel overwhelming, and we appreciate you taking the time to do so.
Our forms are designed to help us provide you with the safest, most comprehensive, and individualized care possible. They include important information about our office policies, your medical and mental health history, current concerns, medications, and other information that helps your provider prepare for your appointment.
By reviewing your information before your visit, your provider can spend less time gathering basic information and more time focusing on you, your concerns, and developing a treatment plan.
All new patient paperwork must be completed and submitted before your scheduled appointment. If your paperwork has not been completed prior to your appointment, you will be asked to reschedule. This allows your provider to dedicate your appointment time to your care while also staying on schedule for other patients.
Please note that our missed appointment/late cancellation policy will apply to appointments that cannot be completed due to incomplete required paperwork.
The good news is that most of these forms only need to be completed once and are simply reviewed or updated annually. Once your initial paperwork is complete, future visits require much less paperwork.
Thank you for your patience and understanding. We know it's a lot to complete, but these forms help us provide the highest quality care from your very first visit.
